If you have lost your copy of your CE or CME certificate we are happy to provide another upon request. Please note, it is necessary to charge a small administrative fee to help cover costs incurred to APSNA to fulfill such a request.

Copies of a CE or CME certificate are available only if:

• You have completed your conference evaluation for the meeting for which you are requesting the certificate
• You have been awarded CE or CME hours for that meeting
• The conference evaluation period is closed

To purchase a copy of your CE or CME certificate from an APSNA meeting held this calendar year select the box below. For example, if you attended the Annual Scientific Conference this year and completed an evaluation, were awarded CEs or CMEs, and evaluations are now closed, this is the box you need to select. Retrieving a certificate may take up to three weeks. The fee is 15.00 and is non-refundable.

To purchase a copy of your CE or CME certificate from any APSNA meeting held any year prior to this calendar year select the box below. For example, if you attended the Annual Scientific Conference any year prior to this year, completed an evaluation, were awarded CEs or CMEs and you need a certificate from previous years, select the box below. Retrieving a certificate may take up to three weeks. The fee is 30.00 and is non-refundable.

Disclaimer:  If you pay to request past certificates, but failed to fill out an evaluation, you will not be given a certificate, and this fee is non-refundable.
